Genetic factors play an important role in the risk of ischemic stroke. Recent studies have shown an association between genetic variants of the apolipoprotein E gene (APOE), the ATP-binding cassette transporter A7 gene (ABCA7) and clusterin (CLU) gene and stroke. However, in a Greek population, the association between APOE, ABCA7 and CLU variants and recurrent stroke is not clear. Therefore, we aimed to evaluate the impact of APOE, ABCA7 and CLU gene variants on the risk of recurrent stroke in a Greek population. In a case-control study, we recruited 143 patients with a history of recurrent ischemic stroke (IS) and 143 controls without previous history of IS. All participants were interviewed using a structured questionnaire. Genomic DNA was isolated from peripheral blood and the APOE, ABCA7 and CLU gene polymorphisms were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). The results showed that ABCA7 K3493R and CLU -493H/T alleles were less frequent in cases than in controls, whereas the APOE epsilon4 was more frequent in cases than in controls. The CT genotype of APOE gene, however, had no significant difference between cases and controls. After adjusting for other variables, multiple logistic regression analysis showed that patients with homozygous APOE epsilon4 had a 2.5-fold higher risk of developing recurrent IS than those ac619d1d87
